According to the images above, Blazar's human host is a member of SKaRD named Hiruma Gento who is rumored to be portrayed by Tomoya Warabino (who portrayed Heart in Kamen Rider Drive. If this is true, he will be the one of the oldest Ultra host).
Hiruma Gentao will transform into Ultraman Blazar, a giant from M421, with the transformation item called the Blazar Brace and the Blazar Stone and his finisher is called the Spiral Blade.
